Medical
Blue Cross, HMO or PPO: 100% Employer paid premium for employee, domestic partner, and covered dependents

Dental
Delta Dental: 100% Employer paid premium for employee, domestic partner, and covered dependents.

Vision
Vision Service Plan: 100% Employer paid premium for employee, domestic partner, and covered dependents.

PTO (Paid Time Off)
20 days per year.

Holidays
9 designated plus 2 personal days per year (1 personal day for the period January 1 - June 30; 1 personal day for the period July 1 - December 31).

Life
1 X annual salary, with maximum of $200,000.

AD&D
Varying benefit levels.

Disability
State Short Term Disability (STD) insurance to be used for a medical leave of absence up to 90 calendar days. After 90 days, Standard Long Term Disability (LTD) in the amount of 60% of monthly earnings, up to a maximum monthly benefit of $10,000. Threshold will make up the difference in disability pay and regular salary for a period of 10 weeks.

401(k)
Employee contributory 401(k) deferral plan.

Stock Option Plan
Stock options may be granted by the Board of Directors under the terms and conditions of the Yalepharma 2005 Stock Option Plan.

Incentive Plan
This Plan has been established to advance the interests of the Company by providing for the grant to Participants of Stock-based and other incentive Awards.

Parental Leave Plan
One business week or five days of paid time off to be used for the birth or adoption of a dependent child within 6 months of the birth or adoption.
